PERSON-CENTRED CARE MEANS THAT THE RESIDENT, THEIR WEALTH OF EXPERIENCES, WELL-BEING, NEEDS AND FEELINGS ARE TAKEN INTO ACCOUNT. PERSON-CENTRED CARE PLACES THE RESIDENT AT THE CENTRE OF THE CARING PROCESS.

Person-Centred Care is central to ensuring that the resident settles into a rewarding and fulfilling life at Chaffey. It assists staff to appropriately meet the individual care needs by listening, learning and sharing what is important to the resident so we can make a difference to every resident, every day.

 

Our residents are encouraged to continue to make their own choices about all aspects of their life at Chaffey where it is safe to do so. We respect and support residents' choices whilst maximising their comfort and safety. We ensure that excellent care is the focal point of our interaction with the residents and their families or nominated representatives. The resident will always come first in the decision making process whilst they reside with us. Meeting the needs of our residents is our community's primary goal.

 

Residents are invited and encouraged to discuss their preferences with our responsive care, recreation and lifestyle staff and all other staff workgroups where needed. Chaffey Aged Care meets and exceeds all Aged Care Accreditation requirements. We operate a robust quality and audit process along with our continuous improvement program, within which your feedback is sought, considered and acted upon. Regular resident consultative meetings and care review conferences are held with Residents and their families or representatives to offer feedback and your participation in those discussions is encouraged.

ACCOMMODATION - HOMESTYLE LIVING ENVIRONMENT

Chaffey Aged Care provides a comfortable living environment including quality furnishings, heating, air conditioning and access to landscaped gardens, various courtyards and outdoor spaces, as well as many newly refurbished and extended amenities.

All of our rooms are single rooms with their own private ensuite. As well as all the essentials, wherever possible, Chaffey Aged Care goes that bit further and we provide our residents with services such as our newly completed barista style café, a volunteer fundraising kiosk, a unisex hair dressing salon, access to our Community Men’s Shed on site, specialised physiotherapy and an Allied Health consulting room  and newly created gym. Entertainment is also provided by community groups that volunteer their talents to the home.There are a number of areas throughout the building for you to socialise, listen to music, watch a movie, have a private meal or refreshments, read a book or look out at the gardens and idyllic river flats. We are proud of our extended outdoor garden area and our lanscaped courtyards, all of which are accessible for you to enjoy.

 

PERSON CENTRED CARE

Our Case Management Care Model produces individualised Care Plans that are developed by Registered Nurses, with additional input from our Occupational and Physiotherapist staff based on individual assessed needs Our care delivery is led by our Director of Nursing, Clinical Care Coordinator and under appropriate delegation to our Registered Nurses, our Enrolled Nurses & Personal Care & Recreation & Lifestyle staff. As a team they assist with all personal care and social support required by residents on a daily basis.

Timeliness and resident safety in medication administration is delivered via an electronic medication management system. Family involvement is critical to medical decisions and sought wherever possible relating to GP Appointments and or Hospital Admissions. Interactive programs are run by our team of qualified and experienced Recreation & Lifestyle Staff and all care is aimed to achieve evidenced based and best practice.

 

SPECIALISED CARE

Our nursing and care staff have expertise in the provision of specialised nursing care such as complex wounds, behaviours associated with dementia and palliative and advanced life care planning and discussions. We access visiting services to support our care such as internal and external allied health professionals, palliative care link nurses, residential in reach nursing support with the Mildura Base Hospital, wound care consultancy, and stomal therapy support.

 

NO NEED TO WORRY

With safe and secure accommodation, easy access to nurse call buttons, 24 hour care, medical and allied health support, security surveillance, fire and safety related technology, residents of Chaffey can feel at ease.

In compliance with Aged Care Standards and Regulations all staff members are required to hold a current police certificate and to attend mandatory training in areas such as Fire Safety and Emergency Procedures, Manual Handling, Infection Control, Food Safety, and Customer Service and additional specifically selected topics aimed at increased skills and knowledge development of staff each year.  This ensures we deliver the best care and support to our valued residents.

 

QUALITY STAFFING

All staff are committed to the Mission and Values of Chaffey Aged Care. Our values are demonstrated in the way we care for the residents. Our staff are qualified and participate in ongoing professional development to maintain contemporary skills and clinical competency. Nursing staff are rostered on duty 24 hours a day, 365 days a year and on call senior nursing and experienced management staff provide back up out of business hours 7 days per week.  We aim to recruit the very best staff based on qualifications, experience, skill, compassion and overall the best organisational and cultural fit.

 

DEDICATED STAFF

Led by the Senior Management Team, Chaffey Aged Care has fantastic staff who deliver excellent care, hospitality services and administration support. We are recognised as industry leaders in the Sunraysia region and we have an effective and proven staffing model and generous rostering to ensure sufficient resources are available to deliver the very best care in order to optimise resident outcomes as well as achieving optimal funding.

 

FOOD SERVICES - HOME STYLE MEALS/CATERING

At Chaffey we believe meals should continue to be an enjoyable experience. Our cooks and the food services team create fresh, delicious and nutritionally balanced meals under the guidance of a visiting dietician.  Residents are offered a choice of menu and a selection of alternate meal choices are available. Our menu is reviewed regularly.  Special dietary requirements for medical, religious or cultural reasons are catered for. We have regular recreation and lifestyle events and we serve party style food and drinks and a Happy Hour every Friday afternoon.

 

RESIDENT LAUNDRY

Personal clothing is laundered on site. It is advised that all clothing is labelled and we can arrange for heat adhesive labels to be fixed to clothing upon admission. Clothing should be suitable to be washed and dried in a commercial washer and dryer. A separate resident's laundry is also provided for those who prefer to still launder their own clothes and belongings.

 

CLEANING & MAINTENANCE

Our cleaners maintain a clean environment in rooms and throughout the home which is second to none. Our laundry staff are responsible for cleaning residents' personal garments including returning clothing to each room.

Our maintenance and garden staff are responsible for the maintenance of the building and maintain the courtyards, gardens and grounds. At Chaffey Aged Care we pride ourselves on our presentation and odour free environment.

 

RECREATION & LIFESTYLE

There is a strong focus on maintaining an active and rewarding lifestyle with a range of activities offered.

A regular activities program is facilitated and delivered by our experienced Recreation & Lifestyle staff. Valuable input and assistance provided by a dedicated team of volunteers ably supported and assisted by our creative and talented administrative staff. Lifestyle programs are designed to be fun, purposeful and meet the individual needs and requests of our residents. Our Recreation & Lifestyle Program includes: craft, knitting, cooking, reading, quizzes, exercises, poetry, carpet bowls, happy hour, karaoke, movies,  gardening, and bingo.  Religious services include: a monthly non denominational church service and special occasion services. Visiting school children, community groups and individuals provide a variety of additional entertainment and events. Regular bus excursions are arranged for the residents, weather permitting. Outings include visits to local places of interest, the local library & RSL Hall, sing-a-longs and shopping excursions to nearby shopping centres as well as local theatre productions at the Arts Centre complex.

 

FAMILY & FRIENDS

Chaffey Aged Care understands that ongoing support and interaction with family and friends is very important. We deliver a family friendly environment with comfortable spacious communal areas and landscaped outdoor garden settings and courtyards and a café that transports our residents to any rival local café experience with its décor, atmosphere and great coffee and eats.

Visitors are welcome and we encourage relatives and friends to be involved in all aspects of the resident's lifestyle and care.

 

PASTORAL CARE

Our aim is to welcome and support you as you settle into your new home and provide ongoing support throughout your stay with us.  We have experienced staff and volunteer members that can offer emotional and pastoral support to residents and families.  All individual beliefs and values are respected. We also have visiting Pastoral Care Professionals from local religious communities.

 

BEAUTY AND GROOMING SERVICES

Access to hairdressing, nail care, and massage are available.  A fee for service may apply in some instances.

 

ENTERTAINING

We welcome you to invite your friends and family for visiting and social events. You and your family may like to celebrate special events in the home. There are a number of options available for your function.  An enquiry or booking may be made via reception or at the cafe.

 

DIETITIAN

You will have access to a visiting dietitian contracted to the home. The dietician will visit with you after you settle in and thereafter as required. A review of your dietary needs and specific dietary guidelines will be provided. Special dietary considerations and supplements are communicated and incorporated into your care plan.

 

PHYSIOTHERAPY

You will have access to our occupational and physiotherapist team as well we welcome external allied health services chosen and contracted by you to come into to the home to deliver services you require. The Occupational and Physiotherapist team will assess your  mobility, transfers and exercise tolerance. A physiotherapy/occupational therapy assessment will be incorporated into your overall care plan which will be developed and enabled with the assistance of the care team and recreation and lifestyle team. The occupational and physiotherapist team will make recommendations for aids and equipment, including appropriate seating and other related care needs as necessary.

 

PODIATRY

A regular visiting podiatry service is provided to our residents and appointments to see them may be made in advance through our nursing care staff.

 

DENTAL, OPTOMETRY & AUDIOLOGY

Chaffey have visiting dental, optometry and audiology service providers. Visits are communicated around the home and bookings can be made through our care staff in advance . You are also welcome to continue to attend practitioners in the community should you wish.

 

OTHER ALLIED HEALTH SERVICES

Speech Pathology, and Aged Persons Mental Health and Social work professionals are accessible to provide consultation on request or on referral.

FEEDBACK

We have feedback forms available throughout our home and we welcome compliments, comments and complaints. This is a great way for us to know how we are doing, and how we can improve our care and services to you as the resident and or support to your family or carer. We have a monthly resident and family consultative meeting. We also conduct individual resident care plan review meetings regularly to ensure we are meeting the care needs and expectations of our resident.
